Quick Answer:

The UMR provider services number is 877-233-1800, an automated (IVR) line for eligibility, benefits and claim status. To reach a live representative, get a passcode on umr.com first and enter it when the IVR prompts you. For prior authorization and predetermination, call UMR CARE at 866-494-4502. Because UMR administers self-funded employer plans, the provider services number on the member's ID card comes first; UMR lists 877-233-1800 for when you do not have the card.

Which UMR Number Should a Provider Call?

Call the provider services number printed on the member's ID card, and use 877-233-1800 when you don't have the card. That is UMR's own instruction on its Contact Us page. For prior authorization, call UMR CARE at 866-494-4502, the number in UMR's prior authorization guide (UM1839, January 2025).

The card matters more at UMR than at most payers. UMR is a UnitedHealthcare affiliate, but it is a third-party administrator (TPA), not an insurer. Its provider FAQ (UM1491, September 2024) explains that employer groups fund their own claims and "no two customer plans are alike." Benefits, prior authorization rules and timely filing all come from the employer's plan document. The card also shows the claims mailing address and the primary network.

UMR provider contact directory showing the provider services IVR at 877-233-1800 for eligibility, benefits and claim status with a umr.com passcode for a live representative, UMR CARE at 866-494-4502 for prior authorization and predetermination, umr.com technical support at 866-922-8266, and payment vendor lines, with a note that the member ID card comes first for self-funded employer plans

UMR Is Not UnitedHealthcare Provider Services

UMR runs its own claims platform and customer service. UnitedHealthcare's provider line and UHCprovider.com do not service UMR claims. If the card shows UMR's logo or says the plan is administered by UMR, use UMR's numbers. For fully insured UnitedHealthcare plans, see our UnitedHealthcare provider services phone guide.

How to Get a UMR Passcode and Reach a Representative

You need a passcode from umr.com to get past the 877-233-1800 IVR to a live person. The IVR answers routine questions by faxback. That faxback also carries a passcode and the number to call, but getting the passcode online first saves a step. UM1491 lists the online steps:

  1. Sign in to umr.com with the practice's provider account.
  2. Enter the member ID number and select the family member.
  3. Choose Summary view from the drop-down and select Search.
  4. Go to "Need additional information on this member?" and select Provider service center. The passcode appears.
  5. Call 877-233-1800, follow the prompts, and enter the passcode.

Plan for the per-call limit. UM1491 says customer service generally helps with three member cases per call, and may allow fewer when call volume is high. If you have a stack of UMR claims, sort them by member and plan several calls.

What a UMR Rep Can and Can't Do on the Phone

UM1491 says UMR's Customer First Representatives can adjust some claims over the phone, depending on the claim details. They can't fix claims with incorrect modifiers or CPT/HCPCS codes; those need a corrected claim. They also can't advise you on how to bill. For the corrected-claim mechanics, see corrected claim vs. appeal.

UMR Prior Authorization Phone Number and Online Requests

Call UMR CARE at 866-494-4502 for prior authorization or predetermination, or submit the request online. UM1839 lists what the representative needs:

  • Patient ID number, name and date of birth
  • Provider and facility name, address and phone number
  • CPT and diagnosis codes
  • Date of service
  • A contact at your office and their phone number
  • Documentation supporting medical necessity

Check whether the service needs review before calling. UM1839 describes the Prior Authorization Requirement Search tool on umr.com: search the patient, select Prior Authorization from the View drop-down, and enter the service details. Results can show "prior authorization is required," "medical necessity review needed / pre-determination recommended," "no coverage for this service," or "no requirements found." Requirements can also depend on age, dollar or visit thresholds, and place of service.

UM1839 says predeterminations and prior authorizations can take up to 15 business days. Results arrive by letter, and a UMR team member may also call. For a denied authorization, the letter lays out the appeal process. To prepare the clinical letter, use our UMR medical necessity letter template. Our prior authorization phone call script covers the call itself.

Peer to Peer Is Requested by Form, Not Phone

UMR's Peer-to-Peer Request Form (UMF0057) is sent to UHC_PeerToPeer_Scheduling@uhc.com through UnitedHealthcare's encrypted email service. The form asks for three time windows when the physician is available and whether an appeal has already been started. Our peer-to-peer review call script covers what to have ready once the reviewer calls.

UMR Mailing Addresses and Payer ID

The claims mailing address is on the member's ID card, so read it from the card rather than a directory. The sample card in UM1491 shows UMR, PO Box 30541, Salt Lake City, UT 84130-0541, and EDI payer ID 39026. A specific employer plan can differ. UM1491 publishes these other addresses:

PurposeAddressSource Note
ClaimsAddress on the member's ID cardSample card shows PO Box 30541, Salt Lake City, UT 84130-0541
Post-service appealsUMR – Claim Appeals, PO Box 30546, Salt Lake City, UT 84130Or submit online through umr.com
Medical records on CDUMR, PO Box 8042, Wausau, WI 54402-8042For paper records, use the fax or address on the request letter
Refund checksUMR, ATTN-Adjustment Team, PO Box 8033, Wausau, WI 54402Include member ID, patient name, date of service and claim number

When UMR denies a claim for medical records, return the request letter with the records. Don't resubmit the original claim. UM1491 notes that UMR denies claims for missing information rather than pending them, so a records request shows up as a denial. For appeals, the UMR appeal form guide walks through the UMF0010 form and the online appeal tool.

What to Have Ready Before You Call UMR

Have these ready before dialing, because the IVR and the rep both route by member and plan:

  • umr.com passcode: required to reach a representative on 877-233-1800
  • Member ID and group number: both on the card; the group number identifies the employer plan
  • Tax ID: the TIN registered on umr.com
  • Claim number or prior authorization transaction number: for status checks
  • Check number or 11-digit FCN: for payment and refund tracking questions
  • The denial reason code: UMR uses standard ANSI claim adjustment codes, per UM1491

A step-by-step version is in our claim status call script and eligibility verification call script. For deadline questions, check UMR timely filing and appeal deadlines first. UMR's limits come from the employer plan and your contract, not a single national number.

Common Mistake

Calling a UMR line for a UnitedHealthcare Shared Services (UHSS) card. UM1491 notes that UHSS claims use the same claim address as UMR, but the card carries no UMR logo or UMR information. Call the number on the UHSS card instead.

Payment Questions: Check Tracers, Remits and EFT

For a missing payment, ask UMR for a check tracer; UMR can start one 30 days after the check was issued. UM1491 says UMR then works with the employer group to confirm whether the check was cashed, which can take up to another 30 days.

Two more payment details explain many "where is my money" calls:

  • The employer sets the payment day. UMR pays claims from each customer's bank account, and the customer chooses which day of the week or month to release payment.
  • EFT is not guaranteed for every payment. EFT enrollment needs approval from the UMR customer group too. Deposits typically land 3–7 days after UMR sends funds to the EFT vendor.

If a paper check arrived without a remit, look it up on umr.com under Claims > Advanced claims > Check number with the 10-digit check number and group number.

How Muni Calls Handles UMR Provider Calls

Muni Calls places configured outbound payer calls, such as UMR claim status or eligibility checks, and returns the result for staff review. Your team confirms the destination number from the member's card, the plan details and the information the rep needs before a call runs. Results include the reference number and representative details when UMR provides them. A demo shows which UMR workflows fit your practice.
