Wellcare's 2026 Medicare Advantage Provider Manual gives participating providers 180 calendar days to submit initial, corrected, and voided claims. The clock starts at the date of service, or the date of discharge for inpatient services. Texas health plans use 95 days. Your provider agreement overrides the manual if it says otherwise. A participating provider has 90 calendar days from the claim denial or utilization management decision to file a reconsideration (appeal). Claim payment disputes follow the timeframe in your contract. A non-participating provider appeals as a Part C reconsideration: 65 days from the denial notice, with a signed Waiver of Liability.
What Is Wellcare's Timely Filing Limit?
For participating providers, Wellcare's Medicare Advantage timely filing limit is 180 calendar days from the date of service, or 95 days for Texas health plans, unless your provider agreement sets a different deadline. The rule is in the "Timely Claims Submission" subsection of Section 7 (Claims and Encounters) of the 2026 Wellcare Medicare Advantage Provider Manual, page 85. It covers initial, corrected, and voided Clean Claims.
Wellcare is Centene's Medicare Advantage brand. One national manual covers every Wellcare MA plan, including the plans formerly branded Allwell and 'Ohana, which moved to the Wellcare name on January 1, 2022. Wellcare By Allwell plans in Arkansas, Missouri, and Texas use the same manual, with their own appeal mailing address.
The manual puts the start date in claim-form terms:
- Professional claims: the "from" date on the CMS-1500 or 837P.
- Institutional claims: the "through" date on the UB-04 or 837I. For inpatient stays, that is the discharge date.
Wellcare Medicaid and Ambetter use different manuals
This guide covers Wellcare Medicare Advantage, including dual-eligible special needs plans. Centene's marketplace brand, Ambetter, sets its windows state by state; see the Ambetter timely filing limits. Wellcare-branded Medicaid plans publish their own state manuals. WellCare of North Carolina's site now says that plan has merged into Carolina Complete Health. Check the manual for the plan on the member's card before you file.

Wellcare Corrected Claim Timely Filing
A Wellcare corrected claim must fit inside the original 180-day (Texas 95-day) window from the date of service. Wellcare does not start a new clock from the remittance. The manual's "Corrected or Voided Claims" subsection says corrected and voided claims "are subject to timely claims submission," and the timely filing rule itself lists "initial, corrected, and voided" claims together. That makes Wellcare stricter than payers that measure corrections from the original payment date.
How to mark a corrected or voided claim, per the 2026 manual (pages 99–100):
- Electronic (837P or 837I): CLM05-3 = 7 (replace) or 8 (void). Put the original Wellcare claim number in Loop 2300 REF02, with REF01 = F8.
- Paper institutional (UB-04): the frequency code is the third character of Box 4 (Type of Bill), and the original claim number goes in Box 64.
- Paper professional (CMS-1500): the frequency code goes left-justified in Box 22, with the original claim number.
Handwritten 'corrected claim' makes it a new claim
If "corrected claim" or "voided claim" is handwritten, stamped, or typed on the form without frequency code 7 or 8 and the original claim number, Wellcare processes it as an original first-time claim. A late "original" claim is denied for timely filing, and you also have a duplicate on file.
Wellcare handles a correction as two transactions. The original claim is reversed and noted "Payment lost/voided/missed." The corrected claim pays under a new claim number, noted "Adjusted per corrected bill." The reversal may show up on a later EOP than the new payment, so reconcile both before you call it an underpayment.
For how other payers count corrected-claim deadlines, see the corrected claim timely filing limits guide.
What Wellcare Accepts as Proof of Timely Filing
For a claim, Wellcare accepts a clearinghouse acknowledgment that Wellcare electronically accepted the claim, or an electronic submission sheet with five specific identifiers. Page 86 of the 2026 manual lists both. The submission sheet must show:
- Patient name
- Provider name
- A date of service that matches the EOB or claim in question
- Prior submission bill dates
- Wellcare's product name or line of business
The manual also lists what does not count: a SNIP rejection letter and a copy of your billing screen. A rejection proves Wellcare refused the claim, not that it accepted it. Rejected claims "should be resubmitted within timely filing limits based on the date of service," so a rejection sitting in a clearinghouse queue keeps using up the original window.
For the appeal itself, Section 9 lists separate examples: a registered postal receipt signed by a Wellcare representative, a similar receipt from a commercial delivery service, or a fax submission confirmation.
Wellcare as secondary payer
The 2026 manual does not publish a separate coordination-of-benefits window. When Wellcare is secondary, bill the primary insurer first, then send Wellcare the balance with the primary payment information. EDI submitters send it in the COB elements; only paper submitters attach the primary EOB. If the primary coverage was retroactively terminated, include proof that you returned the primary payment. Confirm the secondary-claim deadline in your agreement.
Wellcare Appeal and Dispute Deadlines for Participating Providers
A participating provider has 90 calendar days from Wellcare's claim denial or utilization management decision to file a reconsideration, and Wellcare has up to 60 days to decide once it has every required document. Section 9 of the 2026 manual says appeals filed after 90 days "will be denied for untimely filing."
Wellcare runs two provider tracks. The denial reason decides which one to use:
| Track | Use it for | Deadline | What happens to an incomplete request |
|---|---|---|---|
| Reconsideration (Section 9) | Clinical and utilization review denials, such as DN004 (authorization denied), DN019 (denied after medical review), DMNNE (medical necessity not established), and records-required codes | 90 calendar days from the denial | Not reviewed. You have 60 days from a lack-of-information denial to send the records. |
| Claim payment dispute (Section 7) | Untimely filing, incidental procedures, bundling, unlisted or non-covered codes, no prior authorization, benefits exhausted, service exceeds authorization | The timeframe in your contract and any state requirement | Upheld for lack of information |
Both tracks use a Wellcare form that must be completed in full. The provider portal is the preferred channel. Paper disputes go to the address on the back of the member's ID card. Paper reconsiderations go to the Appeals & Grievances address the manual lists for the member's state.
A timely filing denial normally goes through the claim payment dispute track. Attach the clearinghouse acceptance report or the qualifying submission sheet described above. Wellcare lists proof of timely filing as supporting documentation for this track and upholds disputes that arrive without the documents they need.
For a fill-in reconsideration letter, use the Wellcare appeal letter template. To find the right Wellcare form and submission channel, see the Wellcare appeal form guide. Before you file, the claim status call script helps you confirm the receipt date Wellcare has on file.
Non-Participating Providers: Filing and Appeal Deadlines
Wellcare's 2026 MA manual publishes a claim filing window for participating providers only. A non-participating provider appeals a payment denial as a Part C reconsideration: 65 days from the notice, with a signed Waiver of Liability.
Appeals. Under 42 CFR 422.574(b), a provider becomes a party to the organization determination by formally waiving any right to payment from the enrollee for the service. That is the purpose of the Waiver of Liability. 42 CFR 422.582(b) allows 60 calendar days after receipt of the written notice and presumes receipt 5 days after the notice date. CMS's Part C reconsideration guidance states the result as 65 calendar days from the date of the notice. Without the signed waiver, the provider is not a party to the reconsideration.
Claims. The manual says non-participating providers are reimbursed at 100% of the Medicare rate in effect on the date of service. It does not state their filing deadline. Original Medicare's limit is one calendar year after the date of service under 42 CFR 424.44. That rule governs claims to Medicare contractors, and Wellcare's manual does not adopt it. Ask Wellcare Provider Services for the non-participating window and write down the answer, the date, and the representative's name. Don't assume a full year.
For the MA appeal levels after Wellcare's decision (IRE, ALJ, Medicare Appeals Council), see the Centene and Wellcare provider appeal guide and the Medicare Advantage appeal letter template.
How Wellcare Compares With Other Medicare Advantage Plans
Wellcare publishes a specific number for contracted providers, 180 days, where several large MA plans leave the window to the agreement. The Aetna Medicare Advantage timely filing guide explains why Aetna's MA deadline has to be verified against the agreement. The Humana Medicare Advantage timely filing guide covers Humana's one-year published policy.
Two Wellcare details trip up billing teams that work several MA plans:
- Texas is 95 days, not 180. The footnote applies to "Texas health plans." A practice that bills Wellcare in several states needs the member's plan state in its deadline tracker.
- Corrections don't get a new window. A claim paid on day 150 leaves 30 days to correct it. In Texas, a claim paid on day 80 leaves 15.
For every payer on one page, see timely filing limits by insurance company.
Prepare Wellcare Disputes and Reconsiderations With Muni Appeals
Muni Appeals organizes the EOP, denial notice, and submission records you upload, and drafts a reconsideration or claim payment dispute letter for staff review. Your team picks the right track from the denial code, checks the deadline against the manual and your agreement, and attaches the clearinghouse acceptance report or submission sheet Wellcare accepts as proof. Staff then review the draft and submit it through the portal or by mail.
Use Muni to organize documents and prepare drafts. Keep the controlling window, its start date, and the submission confirmation in your team's deadline records.
Frequently Asked Questions
What is Wellcare's timely filing limit for 2026?
For participating providers in Wellcare Medicare Advantage plans, the limit is 180 calendar days from the date of service, or from the discharge date for inpatient services. Texas health plans use 95 days. Your provider agreement controls if it sets a different deadline.
What is Wellcare's timely filing limit for corrected claims?
Corrected and voided claims fall under the same 180-day (Texas 95-day) window, measured from the original date of service or discharge, not from the EOP. Use frequency code 7 to replace or 8 to void, with the original Wellcare claim number in REF02 (qualifier F8) or Box 22 / Box 64 on paper.
How long do I have to appeal a Wellcare denial?
Participating providers have 90 calendar days from the claim denial or utilization management decision to file a reconsideration. Wellcare has up to 60 days to decide once it has the required documentation. Non-participating providers have 65 days from the denial notice and must include a signed Waiver of Liability.
What is Wellcare's timely filing limit for non-participating providers?
The 2026 Wellcare MA manual does not publish a non-participating claim filing window. Original Medicare's one-year limit under 42 CFR 424.44 applies to Medicare contractor claims, and the manual does not adopt it. Confirm the window with Wellcare Provider Services before relying on it.
Does a rejected Wellcare claim stop the timely filing clock?
No. The 2026 manual says rejected claims must be resubmitted within the original timely filing window based on the date of service. A SNIP rejection letter is not accepted as proof of timely filing.
How do I dispute a Wellcare timely filing denial?
File a claim payment dispute, the track Wellcare uses for untimely-filing denials, within the timeframe in your contract. Attach a clearinghouse acknowledgment showing Wellcare accepted the claim, or a submission sheet with the patient name, provider name, matching date of service, prior bill dates, and Wellcare product. Disputes without supporting documentation are upheld.
The deadlines in this guide come from the Wellcare 2026 Medicare Advantage Provider Manual (Sections 7 and 9 and the NJ FIDE SNP addendum), 42 CFR 422.574, 422.582 and 424.44, and CMS's Part C reconsideration guidance, all reviewed November 5, 2026. Wellcare Medicaid and Ambetter plans follow their own manuals and are not covered here. Your Wellcare provider agreement controls where it sets different deadlines. This is administrative guidance, not legal advice.
