Quick Answer:

The Oscar Health provider phone number is 1-855-672-2755. Press Option 4 for Provider Services (claims, benefits, eligibility and administrative questions, Mon–Fri 8 a.m.–6 p.m. local time) or Option 1 for Utilization Management (Mon–Fri 8 a.m.–5 p.m. local time). The same number serves members on Option 2, so pick the provider option. Behavioral health, imaging and other delegated services go to the vendor instead: Optum, eviCore, American Specialty Health or Progeny.

Which Oscar Number Should a Provider Call?

Call 1-855-672-2755 and choose Option 4 unless the question is a utilization review or a service Oscar delegates to a vendor. Oscar runs one shared phone number for members and providers and splits callers by menu option. The 2026 Oscar Provider Manual (last updated September 1, 2026) lists two provider options on that line, each with its own hours.

The number is the same across Oscar's states. Oscar's Texas state supplement points providers to the same (855) 672-2755, Option 4. What changes by state is the vendor that handles a delegated service and the fax number printed on each state's authorization form.

Don't Stop at the Member Menu

Oscar's public website shows 855-672-2755 with Mon–Fri 8 a.m.–8 p.m. hours. That is the member-facing listing. The provider manual gives shorter provider hours: 6 p.m. for Provider Services and 5 p.m. for Utilization Management. Late-afternoon calls about an authorization can miss the UM window.

Oscar Fax Numbers and Mailing Addresses for Disputes and Appeals

Send claim disputes to fax 1-888-977-2062 and clinical appeals to fax 1-844-965-9054. The cover form, not the fax number, tells Oscar what the packet is. Each number below comes from Oscar's current provider cover forms and the 2026 manual.

The network gap exception fax comes from Oscar's Availity prior authorization guide (version 3.0, revised October 2026). The drug PA fax appears in both the manual and Oscar's 2026 provider welcome kit. For which form to use and how to fill it out, see the Oscar Health appeal form guide.

Authorization Faxes Vary by State

Oscar's manual says non-digital authorization requests go on Oscar's Authorization Request Form, faxed "to the number provided on the form," and those forms are published by state at hioscar.com/forms. Oscar prefers Availity Essentials for new authorizations. Pull the form for the member's state rather than reusing a fax number from another market.

Delegated Vendors: When Oscar Isn't the Right Call

Call the vendor first when the service is one Oscar delegates, because the vendor runs that review and its reconsiderations. The manual says that for a delegated service, a call to Oscar ends in a transfer or an instruction to contact the vendor. These are the vendor contacts listed in the 2026 manual's delegated-vendor table:

Two vendor numbers differ between Oscar documents, so match the number to the task. The manual lists Optum at 877-614-0484. Oscar's October 2026 Availity guide tells providers to contact Optum at (877) 855-1317 for behavioral health authorization requests, which must go to Optum directly. Likewise, the manual lists ASH at 800-848-3555, while the Availity guide gives 1-800-972-4226, Option 2 for activating an ASHLink portal account.

The Availity guide also explains how the routing works in practice. Availity redirects an authorization to eviCore or ASH based on the service code, and returns messages such as "Submit to eviCore for review" when a code belongs to a delegated vendor. If you see one of those messages, the vendor owns the request. Calling Oscar won't move it.

Oscar Medical Necessity Denials and Peer to Peer

Request an Oscar peer-to-peer review within seven business days of the authorization denial, using the instructions in the denial letter. The 2026 manual says the provider of record is notified of the opportunity in the denial notice. It does not apply to denials for non-covered benefits.

For vendor-issued denials, the vendor's own process applies. If eviCore, ASH, Optum or Progeny signed the letter, use that vendor's number from the table above. Our peer-to-peer review call script covers what to have open when the reviewer picks up. If the peer to peer doesn't overturn the denial, the Oscar Health prior authorization denial guide walks through the clinical appeal.

Other Oscar Provider Lines and Portals

Most practice questions can be answered without a phone call. Oscar lists these self-service channels and secondary lines:

  • Oscar provider portal (provider.hioscar.com): eligibility and benefits, referrals, claims and payments, and claim disputes.
  • Availity Essentials: Oscar's preferred channel for new authorizations and the only clearinghouse Oscar uses. Payer ID OSCAR. The manual lists claim-status CPIDs 9638 and 7468.
  • Case management referrals: 844-357-6585, per the welcome kit. Refer behavioral health case management to Optum.
  • Payment and remittance options: 888-686-3260, the number the manual gives for Oscar's payment partners.
  • Compliance hotline: 1-844-392-7589, for suspected fraud, waste or abuse.

For transplant claims, the welcome kit routes pricing to Optum Complex Medical Claims (877-801-3507, ext. 3) or Cigna LifeSource, depending on the member's transplant network. Those claims go to the vendor for pricing before Oscar pays them.

What to Have Ready Before You Call Oscar

Have the member and claim identifiers ready before you dial, because Provider Services routes on them:

  • Member ID with the OSC prefix and suffix: Oscar's Availity guide shows IDs in the format OSC12345678-01
  • The member's state and plan name: decides the network, the authorization form and the vendor
  • NPI and Tax ID for the billing provider
  • Claim number or authorization reference: Oscar's Availity guide says requests submitted on and after October 14 use a Case ID
  • The denial or remittance reason code from the Explanation of Payment

For a step-by-step version, use our claim status call script or the eligibility verification call script. If the call is about a deadline, check Oscar Health timely filing limits first.

Common Mistake

Calling Provider Services to argue a clinical denial. Option 4 can explain a claim status or a reason code, but it doesn't decide clinical appeals. For a medical necessity denial, request a peer to peer within seven business days or file the Clinical Appeals form at 1-844-965-9054. The full process is in our Oscar Health appeal guide.
